There are very few good options of medical-insurance for those visiting the US. In the past, I have used plans from http://www.kvrao.org/ & http://www.visitorscoverage.com/ ; my general experience with filing claims has been somewhat mixed. Useful discussions around this topic are here and here. There are similar offered by companies in India that I haven't used.
Recently I came across other options for primary care in the Seattle area (I believe that there are companies offering similar programs across the US). Note that the these plans don't typically cover hospitalization & are best used as supplemental coverage with other insurance plans
Results-Direct.com, local pharmacies (Walgreens or Bartell Drugs) offer reasonably priced immunizations & lab-tests.

- Qliance seems like a very good option for those who require frequent doctor visits. Their plans range from at $44-$129 per month, and includes unlimited doctor visits.
- CarenaMD offers mobile-primary care (doctor visits the patients home) & is very convenient for those patients who aren't very mobile, or prefer care at home. They charge about $2000/year & cover a 12 visits (I think their charges can be prorated, if one needs to cancel before the end of 12 months). I have used their service & really liked it -- the convenience of the doctor visiting home at a short notice & spending 30-60 mins is very helpful. I wish they offered a monthly plan also. (EDIT #1: seems like CarenaMD is now offering additional plans for consumers!)
